Second-born children face two to three times higher risk of serious respiratory infections in infancy compared to their older siblings, and new research from the University of Copenhagen shows these early illnesses can reduce their education, income, and health decades later.
The scenario is familiar to any parent. Your oldest starts daycare. Within days, the entire household is sick. But while a cold might be a minor inconvenience for older children, new Danish research reveals that for babies exposed to viruses brought home by siblings, the consequences can last a lifetime.
Siblings Are the Real Vector
I have watched this play out in countless Danish families over the years. The older child brings home a virus from the institution. The baby gets it worse. Much worse. According to research from the University of Copenhagen, second-born children are hospitalized for respiratory infections at rates two to three times higher than firstborns at the same age. The risk peaks in the first six months of life, when infants are most vulnerable.
“We see that the second child has two to three times higher risk of being hospitalized with respiratory diseases in infancy compared to firstborn children at the same age,” says N. Meltem Daysal, associate professor at the Department of Economics. “It is especially pronounced in the first six months of life, where children are biologically most vulnerable.”
The study followed more than 1.2 million Danish children from birth into adulthood using detailed registry data. Researchers combined differences in birth order with local variations in disease outbreaks to isolate the effect of early infection exposure.
Education and Earnings Take a Hit
The real shock comes years later. Children who suffered serious respiratory infections as infants are less likely to complete gymnasium or university. Their adult incomes are lower. The magnitude is not trivial. The probability of completing higher education drops by approximately 0.4 percentage points. Adult income falls by nearly one percent.
“When we compare siblings, we can see that early disease exposure is associated with lower probability of completing both gymnasium and higher education, and with lower income later in life,” Daysal explains. “A significant part of the income loss can be explained by the fact that illness in infancy affects children’s educational trajectories.”
For expats raising families here, this research hits differently. Denmark’s universal daycare system is a cornerstone of work-life balance. But it also creates a perfect storm for infant exposure to respiratory infections. Your three-year-old goes to vuggestue. Your newborn pays the price.
The Hygiene Theater We Abandoned Too Soon
During the pandemic, Danish authorities distributed detailed guidance. Keep distance from sick family members. Isolate symptomatic children in their own rooms. No hugs, no shared beds. Some researchers publicly urged families to stay away from infected household members, even children.
Today, those recommendations feel like ancient history. COVID-19 is now treated as a routine respiratory infection, alongside influenza. Official restrictions are gone. But the basic advice remains: stay home when sick, wash hands, cover your cough, protect vulnerable family members.
The disconnect is striking. We are told coronavirus is not dangerous for healthy children. Yet research shows that serious early infections, whether RSV, flu, or COVID, can reshape a child’s trajectory. Sundhedsstyrelsen still recommends avoiding close contact with infants when you are sick. But in practice, how many families actually enforce that?
What Happens in the Body
The mechanisms are not fully understood. Some research links severe infant respiratory infections to increased asthma risk, particularly in genetically predisposed children. There is emerging concern about long-term immune and metabolic effects from COVID infections in early childhood. European pediatric organizations emphasize we lack five to ten year follow-up data to assess whether repeated or severe early COVID infections increase risks for autoimmune disease or other chronic conditions.
The Copenhagen study does not isolate specific viruses. It captures the cumulative burden of respiratory illness in vulnerable infancy. The takeaway is clear: what seems like a mild cold in your toddler can trigger serious illness in your infant. And that serious illness can alter outcomes decades later.
Prevention as Investment
The researchers argue that preventing infant illness pays long-term dividends. “Through vaccination, good hygiene, and support for families with both infants and institution children, we can reap benefits that reach far beyond the healthcare system,” Daysal says.
For families navigating Danish life, this means practical choices. Should your sick four-year-old skip daycare an extra day? Should grandparents avoid visiting when older siblings have symptoms? Should you enforce handwashing before anyone touches the baby? These are not just inconveniences. They are investments in your youngest child’s future health, education, and earning potential.
Denmark has moved on from pandemic-era caution. But for families with infants and older siblings, the risks have not disappeared. The virus your older child brings home today could still be shaping your baby’s life in 2050.
